We provide residential services and supports to individuals with intellectual and developmental disabilities within the framework of the Human Care Agreement. We accept persons at the moment for In-home support and Companion Services. The full range of services and supports we offer are expanding as we look forward to adding more services soon.

Companion Services
Companion services provide non-medical assistance and supervision to support a person’s
goals, desires, and needs as identified in the person’s Individual Support Plan (ISP), and
reflected in his or her Person-Centered Thinking and Discovery tools. Goals may be related to
the person’s safety, promotion of independence, community integration, and/or retirement.
Companion services may be provided in a person’s home or in the community.
In-Home Support Services
In-Home Support Service is provided to persons living in their own home or living in their family
member’s home. In-Home Support services are blended services that provide habilitation,
personal care and other support services to the person in their home. These services assist the
person to reside successfully in their home as their primary place of residence.
Individualized Day Supports (IDS)
IDS are services tailored to the individual's evaluated requirements and self-selected objectives.
This service offers essential habilitation help in the community to enhance an individual's
community integration by developing specific skills required for independence and participation
in community activities beyond the home. The programs are designed and managed to improve
community integration results through tailored, pre-planned activities and opportunities that
take place in integrated and inclusive community settings. Day Supports focus on developing
skills to enhance community participation, involvement, and self-determination. This will be
achieved by maintaining a staff-to-client ratio of 1:1 or 1:2.
Respite (Daily - Out of the home) (Hourly - In the home)
Family members and primary caregivers can take a break thanks to respite services. Our respite
services are created to allow them to take part in planned or unplanned time away from the
person while also ensuring that the person continues to receive all of their services.
Host Home
Samaritan Supports personal supports and assistance to persons in the host home, includes,
but are not limited to the following:
Room and board
Assistance with interpersonal and social skills
Assistance with household chores
Assistance with mobility
Assistance with eating and food preparation
Assistance with personal hygiene
Assistance with dressing
Assistance with motor and perceptual skills
Assistance with enhancing the person’s opportunities for social, recreational, and religious
activities utilizing community resources
Assistance with ensuring that the person’s adaptive equipment is clean and appropriately
functioning
Day Habilitation in the Community
Samaritan Supports helps individuals with disabilities develop, maintain, or improve skills and functioning for daily living, often in a non-residential setting, focusing on areas like self-help, socialization, and community integration.
